Our school nurse is on-site to help keep our students and staff safe and healthy. The district's nurse practitioner also provides physical exams to students at no cost. Prior to performing a physical exam, legal guardians must complete a parent permission form. Everyone has to sign this form when they register their child for school. Please check with the nurse if you have any questions/concerns about this form. The form also grants the nurse permission to discuss health information with your child's physician, if medically necessary.
